This statement sets forth Dental Club One's Privacy Policy and describes the practice Dental Club One follows with respect to all visitors

Dental Club One has implemented the necessary technology to secure ALL information that is transmitted between your

SSL is a communications protocol used on the Internet to establish a secure connection with a web server. This secure connection relies on
public key cryptography and requires a digital certificate signed by a trusted authority like VeriSign, which issued a certificate to Dental
Club One. When a browser requests an SSL protected link from our web server, the browser and the server conduct security transactions. During
the transactions, the server sends its certificate to the browser, the browser verifies its authenticity, and the server and browser exchange
the session keys to be used for bulk encryption of data transmissions between the server and the browser.

Once enrolled, all transactions transmitted via Dental Club One are securely delivered using 128 Bit encryption
technology. All transactions are approved and assured by Wells Fargo Bank and
the Federal Reserve Bank of the United States. Dental Club
One takes the security of personal and payment information seriously. This is why Dental Club One only offers the best protection of your
information on the World Wide Web.
Protected
Profile Information
Generally, your profile information (such as city and state) in the enrollment form is used by Dental Club One to make Dental
Club One's Web site more user-friendly. Dental Club One may also profile your Web site activity in order to accurately track your
account and provide you with Dental Club One products and services that correspond to your interests. Dental Club One may also compile
data in aggregate form so that we may better understand the users that are visiting our site. For example, we may produce reports on
the most popular search terms by collecting general search term data based on individual searches. Aggregate data is anonymous and
does not contain any personal information that identifies a user. Simply stated, we use profile information to make our site more
valuable to you the consumer.
Automatically Collected Information
As is true of most web sites, we gather certain information automatically and store it in log files. This information includes internet
protocol (IP) addresses, browser type, internet service provider (ISP), referring/exit pages, operating system, date/time stamp, and
click-stream data.
We use this information, which does not identify individual users, to analyze trends, to administer the site, to track users' movements
around the site and to gather demographic information about our user base as a whole.
We do link this automatically collected data to personally identifiable information. IP addresses are tied to personally identifiable
information to segregate customer types for analytical purposes. When you use Opinion Lab, email addresses are not automatically
collected but may be optionally submitted by user. Dental Club One will use this email address and any personal information collected
for the purpose of responding to the user's request.
Children
Children (persons under the age of 18) are not eligible to use this Web site unsupervised and we ask that children do not register
for an account or submit any personal information to us. By using this site unsupervised, and/or registering for an account, you warrant
that you are 18 years of age or older.
Change of Address or Contact Information
If your identifiable personal information changes, or if you no longer desire our service, you may correct, update or terminate your
information through the Member Services page. Contact us via email, telephone, Fax or postal mail and your request will be processed.
Discount Dental Card
Please note that our Gold plan is not insured and is not an insurance product. CAREINGTON does not make
payments directly to medical services providers. You are obligated to pay for all health care services. You will receive discounts
for medical services at certain health care providers who have contracted with the plan. This program is administered by CAREINGTON.
Limitations
This is a discount program. This is not an insurance plan. Dental Club One, and CAREINGTON cannot guarantee specialty
care in all areas. In cases in which you are referred to a participation specialist, you will generally receive 20% off their
normal fees. Please verify such discounts with each individual provider. Work in progress, after enrollment on the plan,
must be completed by the provider who started the work. Any procedures performed by a non-participating provider are not included.
Dental Club One and CAREINGTON cannot guarantee the continued participation of any provider. Not all types of providers
may be available in your area. Some providers may charge for missed or broken appointments if no prior notice is given. It is the
member's responsibility to verify that the provider is a participating provider. This plan does not include all procedures which might
be provided. Any procedure delivered which is not listed on the Plan Schedule may cause additional cost to be incurred by the member.
The dollar amount specified adjacent to each procedure may not be the only cost incurred for a given treatment because the treatment
may require more than one procedure.
Cookies
A cookie is a small text file that is stored on a user's computer for record-keeping purposes. We use cookies on this site. We do not
link the information we store in cookies to any personally identifiable information you submit while on our site. We use session cookies
to make it easier for you to navigate our site. A session ID cookie expires when you close your browser.
Changes in this Privacy Statement
If we change our privacy policy, we post will changes this privacy statement, the homepage, and other places we deem necessary. We
reserve the right to modify this privacy statement at any time, without the notification to all our members, but if a change were to
be implemented, Dental Club One will do its best to keep members informed. So please review it frequently. If we make material changes
