no waiting, no forms, and no referrals. You are free to visit any of the participating dentists

How do I schedule an appointment with a participating dentist or specialist? |

Once you receive your membership card, you simply call a participating dentist to make
an appointment and tell them you are a member of the plan. Although you know us as Dental Club One the dentist or
specialist is part of the Network and uses the network name to access the reduced fees. |

When will I receive a membership card? |

Every member of our network has been carefully selected to meet our network criteria. Each dentist and specialist meet highly selective credentialing standards based
on education, background, ADA certification, licenses and other regulatory requirements. |

A membership includes all household family members. This includes your entire household, your entire family, or family members. You determine who is considered part of your household. |

What if I need to change my dentist? |

Examples of claim related information commonly requested: |

- X-ray Requests: Crown and crown build-up procedures require pre-operative x-rays to be submitted with the claim.
- Prior Placement Information: When a new prosthetic appliance (denture or partial denture) is being made to replace
an existing appliance; the date of the initial appliance is necessary to be able to determine the eligibility for this service
and the reason to replace the appliance is required. This would also apply to replacement crowns on individual teeth.
- Missing Tooth Information: A partial denture/bridge claim submission requires the extraction date
of the missing tooth/teeth and also the tooth numbers of the missing teeth.
- Coordination of Benefits: If another insurance carrier is responsible as the primary payer for
a claim; this information should be submitted with the claim.
- Service Dates: Crown and bridge procedures require more than one appointment to complete the
required services. The appointment that the procedure is initiated is the 'prep' date for the service. The appointment that the
crown and/or bridge is cemented in place is the 'seat' date. The 'seat' date is the date that should be submitted on the claim form
as the service date for the procedure.
- Procedure codes: Only valid American Dental Association CDT4/5 procedure codes should be
submitted on the dental claim forms.

Do I need to call Dental Club One if I want to change providers? |

You are free to change network dental providers at any time anywhere in the country

What procedures are included under the Discount Dental Card plan? |

In addition to standard preventative dental procedures, the benefits also include savings on
specialty procedures including orthodontics, periodontics, root canals, crowns, bridgework, fillings, dentures and veneers.

When do I renew my dental plan? |
| A: |
If you select the monthly ACH billing method for payment, no action is required to renew your
dental plan. If you select the annual prepayment method, a renewal notification and billing statement will be mailed to Dental Club One
in advance of your anniversary date. |

Renewal/Cancellation/Termination? |
| A: |
This Plan renews at each yearly anniversary of the effective date. Company and Subscriber
each have the right to terminate the Plan with prior written notice. Please consult the Individual Dental Agreement for details
concerning renewability, cancellation and termination. |

What is the most common cause of dental claims delays? |
| A: |
Occasionally, we must request x-rays, periodontal charting and/or other claim related
information to complete the processing of a claim if it is not initially submitted with the claim. If it is necessary to request
additional information on a claim, the dental processors will request this information from the dentist and they will also notify
the insured of the request. |
